Healthcare Management Planning Risk Assessment Questionnaire

This questionnaire is intended to be sent out during the risk assessment and fiscal audit planning period. It is sent to financial and operational managers throughout an organization with the intention of gathering their opinions on a number of predetermined potential audit areas within the company. The completed questionnaires are returned to the internal audit group and used to build a portion of the year's audit plan.

This questionnaire is intended as a sample and modification and customization will be required to suit each organization. Although it is healthcare-specific, the shell and function of the tool can be easily customized and modified for other industries.
