This sample checklist can be used for a risk assessment to identify failure scenarios and the likelihood and severity of different risk areas.

This checklist covers environmental risk, man-made risk, business risk and information technology (IT) risk. Environmental risks covered include arctic freeze/ice, drought, epidemic, hurricane, volcanic activity and winter storms. Man-made risks covered include air pollution, aircraft crash and neighboring business risk. Business risks covered include acquisition target, credit risk and customer service downtime. IT risks covered include backup process/media failure, computer viruses, email downtime and frequent equipment failures.
