It is sent to financial and operational managers throughout an organization with the intention of gathering their opinions on a number of predetermined potential audit areas within the company. The completed questionnaires are returned to the internal audit group and used to build a portion of the year's audit plan.

This questionnaire is intended as a sample and modification and customization will be required to suit each organization. Although it is healthcare-specific, the shell and function of the tool can be easily customized and modified for other industries.
